When complete, the form may be exported to PDF (via the "EXPORT PDF" button at the bottom of the page) and emailed to CASEM along with any other relevant information (you may attach a CV of up to 6 pages).
You may also save your form locally (via the "SAVE FILE" at the bottom of the page). This saves a data version (.json file) of your completed form to your computer for future editing and all future games application submissions. (Please check your local default downloads folder for the file) *Please note that in an effort to have users control and protect their privacy all information in the completed form resides on your local computer and NOT on the CASEM server.
To EDIT your saved form, return to this page and click "UPLOAD DATA FILE" to open the saved data file from your local computer, and click on the field you wish to edit. When complete either export to PDF or save your form locally once again. When you save locally, the form will always save a NEW version of your file. It will not overwrite an older file.
IMPORTANT:
YOU MUST SAVE YOUR FORM TO YOUR LOCAL COMPUTER IF YOU WISH TO ARCHIVE THE FILE FOR FUTURE USE. You cannot edit the saved PDF file.
CLOSING THE BROWSER WINDOW WITHOUT SAVING WILL RESULT IN THE LOSS OF ALL WORK.
Please send ONLY the exported PDF file to CASEM via email. Do NOT send the saved data file. The saved data file is for individual archival and editing purposes only.
